#1. Have you had missing or lost time of any length, particularly of one or more hours?
#2. Have you awakened to find yourself seemingly paralyzed or restrained for no reason and felt an unexplainable presence near you?
#3. Have you found any unusual scars or marks with no possible explanation on how you received them? If so, did you have an adverse emotional reaction to them? (i.e. A small scoop indentation or a straight line scar, scars in roof of mouth, in your nasal passages, behind or in your ears, or genitals, etc.)
#4. Have you seen balls of light or flashes of light in your home or other locations?
#5. Have you had memories of flying through the air which did NOT seem like a dream?
#6. Have you had a "marked memory" that would not go away? (i.e. alien faces, medical examinations, needles, lying on a table, strange appearing babies, pain probes in sensitive areas of your body etc.)
#6. Have you seen beams of light outside your home, or shining into your room through a window?
#7. Do you have repeated dreams of UFOs, or small, grey alien beings with large black eyes?
#8. Have you had one or more "UFO" sighting or encounter at anytime your life?
#9. Have you experienced feelings of cosmic awareness or a great concerned interest in ecology, the environment, vegetarianism. Or do you find yourself very socially conscious?
#10. Do you have a strong sense that you have an important mission or task to perform, without knowing where this compulsion came from or what your mission is?
#11. Have unexplainable events occurred in your life that have left you feeling strangely anxious afterwards?
#12. A question for the ladies: Have you had a false pregnancy or missing fetus? (pregnant, and then not)
#13. Have you ever awakened in a place other than where you went to sleep, or don't remember ever going to sleep? (i.e. upside down in bed, or in your car)
#14. Have you had dreams of eyes such as animal eyes (like an owl or deer), or remember seeing an animal staring at you?
#15. Do you experience an uncontrollable fear when your movements are watched by a household pet?
#16. Do you have a strong adverse reaction to films or even drawings of aliens or UFOs?
#17. Do you have inexplicably strong phobias ie: snakes, spiders, large insects, certain sounds, bright lights, heights or being alone?
#18. Do you experienced frequent self-esteem problems ie: your appearance, abilities, ideas or decisions that you make throughout your life?
#19. Have you ever seen any other person in your presence become paralyzed, motionless or seemingly frozen in time, especially someone you sleep with?
#20. Have you ever awakened with marks, burns or bruises which appeared during the night with no explanation how they possibly got there?
#21. Has someone in your life ever claimed to have witnessed a spaceship or alien being near you or perhaps witnessed that you had been missing for a substantial period of time?
#22. Have you, at any time, discovered blood stains or any other unusual stains on your bedsheets or pillow, with no clue how it got there?
#23. Do you find yourself obsessed with the subject of UFOs and or alien contact resulting in a compulsive urge to research and study as much material on the subject as you can possibly find; such as books, videotapes, the internet, etc.?
#24. Have you at times felt compelled to travel to an area previously unknown to you, or perhaps visited a place or location that you had never been to before, but yet knew everything about that place?
#25. Do you frequently get the feeling that you are being watched, by more than one set of eyes, particularly at night?
#26. Do you have recurring dreams of floating through a closed window, door or a solid wall?
#27. Have you ever seen a strange fog or mist that had no logical explanation for being there?
#28. Do you ever hear strange humming, hissing or pulsing sounds that you can't identify nor locate the source of?
#29. Do you experience frequent, nose bleeds for no reason that recur for days or even weeks at a time and then stop for months or even years before happening again?
#30. Do you ever wake up in the morning to find that your genitals are unusually sore, and find no explanation why?
#31. Have you often sufferred from excessive back or neck problems, Particularly with your T-3 vertebrae going out on you?
#32. Have you had chronic sinusitis or nasal problems?
#33.Have any electrical items around you ever gone haywire or oddly malfunctioned for no reason; such as street lights going out as you walk under them, TV's and radios affected as you move close, etc.
#34. Do you have faint memories of seeing a hooded figure in or near your home, or even next to your bed at night?
#35. Do you, on occassion hear a constant or sporadic ringing in one or both of your ears that you can't seem to escape?
#36. Do you have an intense fear of doctors or undergoing a simple medical examination to the point of fabricating any excuse that you can think of to avoid medical treatment?
#37. Do you suffer from insomnia or sleep disorders which are puzzling to you?
#38. Do you have nightmares about doctors or medical procedures?
#39. Do you have frequent or sporadic headaches, especially in your sinus cavity, behind one eye, or in one ear?
#40. Do you often feel like you are going crazy for even thinking about such unbelievable things like; Aliens, Abductions, UFOs, etc?
#41. Have you ever had any paranormal or psychic experiences, including indepth intuition?
#42. Are you prone to be compulsive or act obsessively?
#43. Do you believe that you have ever received or channeled telepathic messages from extra-terrestrials?
#44. Have you ever been terrified of your bedroom closet, now or as a child?
#45. Have you been having problems maintaining a serious relationship because you have this mysterious "feeling" that if you become involved with someone it will interfere with "something" important that you must do?
#46. Do you have a difficult time trusting other people, especially authority figures?
#47. Do you have vivid dreams of mass destruction, catastrophies, or impending doom surrounding you?
